Social Security Card Name Change: Documents Required
To update your name on a Social Security card, you generally need: evidence of the name change plus proof of identity (and, in some cases, proof of status/citizenship).
1) Evidence of name change (choose one)
- Marriage certificate
- Divorce decree/certificate
- Court order for the name change
2) Proof of identity (choose one)
- U.S. driver’s license
- State-issued non-driver ID
- U.S. passport
If you do not have a primary ID
The SSA may accept other current documents that show your name plus identifying information (date of birth or age). A photo is preferred when available.
- U.S. military ID (active, retired, National Guard, or dependent)
- Certificate of Naturalization or Certificate of U.S. Citizenship
- U.S. Indian Tribal Card
- U.S. government employee ID (for example, Form OF-55)
- School ID card (name, date of birth, and photo)
Important reminders
- Mail original documents; photocopies are typically not accepted.
- Include your name-change document even if your ID is in your previous name.
