Which Identification Document Can I Use for My Child?
For a child’s Social Security card request, the SSA generally needs identity evidence for the child and, in some cases, supporting records.
Primary identity documents (child)
- U.S. passport
- State-issued non-driver ID (when available)
Other documents that may be accepted
When a primary document is not available, the SSA may accept other current records that show the child’s name plus identifying information (date of birth or age). Examples include:
- School record or school ID (when applicable)
- Certified medical record extract or immunization record (SSA rules apply)
- Certified childcare/preschool record (current or prior year)
- Religious record (baptismal record, naming certificate, etc.)
- Adoption decree (for recent adoptions)
Important reminders
- Mail original documents; photocopies are typically not accepted.
- Use a valid return mailing address so documents can be returned safely.
